How to file a successful claim: documentation checklist

Most claim rejections aren't because the claim is invalid — they're because of incomplete or late documentation. This is a checklist you can use across health, motor, term, and home insurance claims.

Universal documents

  1. Filled and signed claim form (each insurer has a different one)
  2. Original policy document or policy number
  3. Government-issued ID (Aadhaar, PAN, passport)
  4. Bank cancelled cheque for direct credit

Health-specific

  1. Hospital discharge summary (original)
  2. All bills, prescriptions, lab reports
  3. Pre-authorisation form (for cashless)
  4. FIR or police report (for accidents)
