How to file a successful claim: documentation checklist

Most claim rejections aren't because the claim is invalid — they're because of incomplete or late documentation. This is a checklist you can use across health, motor, term, and home insurance claims.
Universal documents
- Filled and signed claim form (each insurer has a different one)
- Original policy document or policy number
- Government-issued ID (Aadhaar, PAN, passport)
- Bank cancelled cheque for direct credit
Health-specific
- Hospital discharge summary (original)
- All bills, prescriptions, lab reports
- Pre-authorisation form (for cashless)
- FIR or police report (for accidents)
